Name Change

Please include a copy of Marriage Certificate, Divorce Decree, or other official documentation along with your Change of Name Form.

Ownership Change

Interests can be transferred in a multitude of ways, including by Assignment, Conveyance, Warranty Deed, Quitclaim Deed, etc. Azul Resources requires a copy of the recorded instrument in order to process a change of ownership. Please email or mail a copy of all recorded documents to OwnerRelations@azulresources.com for processing.

W-9 Form

New owners will also need to submit a W9 Form for future payments.
